Additional Services
*For Sex-Offender Risk Assessments: Documentation requested by this provider as well as 50% of cost, non-refundable, is required prior to setting an appointment. A minimum of a 48-hour notice (business days) is required for cancellations. After the first cancellation failing to meet this requirement (including failure to attend), will result in the request for the remaining 50% of cost (non-refundable) prior to rescheduling. There will only be one opportunity to reschedule.
